Do not submit medical information—including symptoms, diagnoses, medications, treatment details, insurance information, or emergency requests—through the booking form.
How information is used
Information may be used to schedule and conduct the introduction, send appointment communications, protect the booking service, respond to requests, and measure completed bookings from advertising.
Service providers
The booking experience may use Calendly, Microsoft Outlook, Zoom, Meta advertising tools, and the website hosting provider. These providers process information under their own privacy practices.
